Overview
- Lead the Case Management department, ensuring effective patient care transitions and cost-efficient operations.
- Collaborate with senior leadership to align departmental goals with organizational objectives.
- Oversee clinical, financial, quality, and HR management within the department.
- Implement and maintain programs to enhance patient management and care continuity.
- Manage a team of RNs, Social Workers, and administrative support staff.
- Ensure compliance with healthcare regulations and standards.
- Promote education and professional development within the department.
- Monitor and evaluate departmental performance metrics and outcomes.
- Foster a culture of excellence and continuous improvement in patient care.
Key Responsibilities & Duties
- Develop and implement strategic plans for the Case Management department.
- Coordinate patient care transitions across the continuum of care.
- Supervise and mentor departmental staff, ensuring high performance and engagement.
- Collaborate with interdisciplinary teams to optimize patient outcomes.
- Monitor departmental budgets and financial performance.
- Ensure compliance with healthcare laws, regulations, and accreditation standards.
- Conduct regular performance reviews and provide constructive feedback to staff.
- Implement quality improvement initiatives to enhance patient care and departmental efficiency.
- Represent the department in organizational meetings and initiatives.
Job Requirements
- Bachelor of Science in Nursing (BSN) required; advanced degree preferred.
- Current RN license in New Mexico or compact license mandatory.
- Minimum of 3 years of experience in utilization and case management.
- At least 2 years of leadership experience in a healthcare setting.
- Certification as CCM or ACM preferred; InterQual certification desirable.
- Strong knowledge of healthcare regulations and standards.
- Excellent communication, organizational, and leadership skills.
- Ability to manage budgets and financial performance effectively.
- Proven track record in implementing quality improvement initiatives.
