Job Description

Overview

  • Provide exceptional care to residents in a long-term care facility as a Licensed Practical Nurse (LPN).
  • Work closely with Registered Nurses and Clinical Care Managers to ensure patient safety and satisfaction.
  • Perform charge nurse responsibilities as assigned, including supervising and guiding nursing staff.
  • Maintain compliance with healthcare regulations and facility policies in all nursing practices.
  • Commit to rotating weekend shifts and a minimum of three shifts per week.
  • Engage in professional development opportunities to enhance nursing skills and knowledge.
  • Contribute to a collaborative and supportive work environment within the healthcare team.
  • Participate in in-person interviews as part of the hiring process.

Key Responsibilities & Duties

  • Administer medications and treatments as prescribed by healthcare providers.
  • Monitor patient health and report changes to the Registered Nurse or Clinical Care Manager.
  • Document patient care activities accurately and maintain medical records.
  • Assist in developing and implementing individualized care plans for residents.
  • Provide emotional support and education to patients and their families.
  • Ensure adherence to infection control protocols and safety standards.
  • Supervise nursing assistants and other support staff during assigned shifts.
  • Participate in team meetings and contribute to quality improvement initiatives.

Job Requirements

  • Valid New York State Licensed Practical Nurse (LPN) license.
  • Completion of an approved practical nursing program.
  • Basic Life Support (BCLS) and IV certification required.
  • Minimum of one year of experience in long-term care settings.
  • Experience working with geriatric populations preferred.
  • Commitment to rotating weekend shifts and minimum weekly shift requirements.
  • Strong communication and interpersonal skills to interact effectively with patients and staff.
  • Ability to work on-site and adhere to facility schedules and policies.
