Key Responsibilities & Duties

  • Deliver direct patient care under the supervision of a registered nurse, adhering to professional nursing standards.
  • Coordinate patient care plans, ensuring compliance with established guidelines and practices.
  • Accurately document patient services and updates in electronic medical records systems.
  • Provide education and counseling to patients and families to promote understanding and comfort.
  • Administer pain and symptom management treatments following care plans.
  • Maintain confidentiality and adhere to organizational policies and procedures.
  • Participate in interdisciplinary team assessments and contribute to quality improvement initiatives.

Job Requirements

  • Graduate of an accredited practical nursing program with a valid NYS Practical Nurse license.
  • Preferred one year of clinical nursing experience, ideally in Med-Surg or hospice settings.
  • Proficiency in electronic medical records systems and critical thinking skills.
  • Commitment to the hospice philosophy of care and professional standards.
  • Availability to work weekends and holidays as required, with flexible scheduling.
  • Strong teamwork and communication skills to collaborate effectively with diverse populations.
  • Participation in ongoing training and professional development programs.
  • Associate of Applied Science (AAS) degree required.
