Job Description

Overview

  • Provide trauma-informed case management for court-mandated individuals in Brooklyn Felony Diversion programs.
  • Conduct psychosocial assessments and draft reports for court recommendations.
  • Collaborate with interdisciplinary teams to ensure high-quality clinical services.
  • Monitor compliance with treatment mandates and coordinate care with providers.
  • Support clients in crisis and assist with immediate needs.
  • Develop and implement evidence-based interventions and policies.
  • Maintain accurate client records in electronic databases and paper files.
  • Participate in community events and interdisciplinary case conferences.
  • Provide clinical guidance to team members on trauma-informed practices.

Key Responsibilities & Duties

  • Conduct comprehensive psychosocial assessments and provide court recommendations.
  • Manage case compliance and coordinate with service providers.
  • Draft and present participant reports to court stakeholders.
  • Collaborate with community organizations for client referrals and services.
  • Provide crisis intervention and immediate support to clients.
  • Assist clients in navigating court processes and compliance issues.
  • Participate in team meetings, trainings, and grant reporting activities.
  • Accompany clients to community appointments as needed.
  • Support the development of clinical policies and procedures.

Job Requirements

  • Master of Social Work (MSW) degree with LMSW licensure in New York State.
  • Minimum of 3-4 years of experience in criminal justice or related fields.
  • Proficiency in biopsychosocial assessment and trauma-informed practices.
  • Strong written and verbal communication skills for interdisciplinary collaboration.
  • Experience with justice-involved communities and diverse client populations.
  • Bilingual proficiency in English and Spanish is strongly preferred.
  • Ability to manage case compliance and coordinate with multiple stakeholders.
  • Familiarity with electronic database systems for client record management.
  • Commitment to attending community events and interdisciplinary meetings.
