Job Description
Overview
- Provide skilled physical therapy services to patients, adhering to infection control practices and agency policies.
- Conduct initial and ongoing physical therapy assessments, focusing on functional limitations and patient-centered goals.
- Utilize telehealth for pulmonary and cardiac rehabilitative services, assessing therapy responses effectively.
- Collaborate with home care physicians and clinical teams to maintain and update physician orders.
- Demonstrate proficiency in OASIS evaluations, achieving high accuracy in chart reviews.
- Manage case oversight and coordination, ensuring efficient resource use and avoiding re-hospitalization.
- Provide timely documentation, including OASIS data and mandated federal and state notices.
- Support and promote the mission of the client, attending mandatory in-services and team meetings.
Key Responsibilities & Duties
- Deliver physical therapy services using motivational interviewing and chronic disease management strategies.
- Perform evaluations to assess functional limitations, pain, and other conditions as per state practice standards.
- Develop and document patient-centered plans of care with clear intervention goals.
- Ensure timely and accurate documentation of visits, discharges, and functional assessments.
- Collaborate with interdisciplinary teams to achieve patient goals efficiently.
- Provide case management oversight, prioritizing and delegating patient visits effectively.
- Maintain compliance with federal and state regulations, including Medicare notices and functional assessments.
- Participate in team meetings and mandatory training sessions to ensure professional development.
Job Requirements
- Associate of Applied Science (AAS) in Physical Therapy or related field.
- New York State License and current registration as a Physical Therapist.
- Ability to comply with New York State regulatory requirements for patient care.
- Proficiency in OASIS evaluations and documentation practices.
- Experience in telehealth services for pulmonary and cardiac rehabilitation preferred.
- Strong case management and coordination skills to achieve patient-centered goals.
- Flexibility in scheduling and commitment to meeting patient needs.
- Previous experience as a Physical Therapist is preferred but not mandatory.
