Job Description

Overview

  • Provide skilled physical therapy services to patients, adhering to infection control practices and agency policies.
  • Conduct initial and ongoing physical therapy assessments, focusing on functional limitations and patient-centered goals.
  • Utilize telehealth for pulmonary and cardiac rehabilitative services, assessing therapy responses effectively.
  • Collaborate with home care physicians and clinical teams to maintain and update physician orders.
  • Demonstrate proficiency in OASIS evaluations, achieving high accuracy in chart reviews.
  • Manage case oversight and coordination, ensuring efficient resource use and avoiding re-hospitalization.
  • Provide timely documentation, including OASIS data and mandated federal and state notices.
  • Support and promote the mission of the client, attending mandatory in-services and team meetings.

Key Responsibilities & Duties

  • Deliver physical therapy services using motivational interviewing and chronic disease management strategies.
  • Perform evaluations to assess functional limitations, pain, and other conditions as per state practice standards.
  • Develop and document patient-centered plans of care with clear intervention goals.
  • Ensure timely and accurate documentation of visits, discharges, and functional assessments.
  • Collaborate with interdisciplinary teams to achieve patient goals efficiently.
  • Provide case management oversight, prioritizing and delegating patient visits effectively.
  • Maintain compliance with federal and state regulations, including Medicare notices and functional assessments.
  • Participate in team meetings and mandatory training sessions to ensure professional development.

Job Requirements

  • Associate of Applied Science (AAS) in Physical Therapy or related field.
  • New York State License and current registration as a Physical Therapist.
  • Ability to comply with New York State regulatory requirements for patient care.
  • Proficiency in OASIS evaluations and documentation practices.
  • Experience in telehealth services for pulmonary and cardiac rehabilitation preferred.
  • Strong case management and coordination skills to achieve patient-centered goals.
  • Flexibility in scheduling and commitment to meeting patient needs.
  • Previous experience as a Physical Therapist is preferred but not mandatory.
