Overview

  • Provide comprehensive primary care services to patients within a value-based care model, focusing on holistic health outcomes and patient-centered care.
  • Collaborate with multidisciplinary teams to deliver coordinated care and ensure optimal patient health outcomes.
  • Engage in leadership roles within the medical center, promoting a positive culture and driving quality improvement initiatives.
  • Utilize evidence-based practices to address acute and chronic health conditions, emphasizing preventive care and lifestyle interventions.
  • Manage a patient panel of approximately 400 individuals, ensuring personalized and accessible care delivery.
  • Participate in professional development and training programs to enhance clinical and leadership skills.
  • Adhere to regulatory compliance standards and organizational policies to maintain high-quality care delivery.
  • Contribute to the recruitment and onboarding of new healthcare providers, fostering a collaborative team environment.

Key Responsibilities & Duties

  • Diagnose and treat acute and chronic illnesses in older adult patients, ensuring comprehensive care management.
  • Develop and implement individualized care plans, coordinating with specialists and support teams as needed.
  • Lead care teams, including medical assistants and care coordinators, to deliver efficient and effective patient care.
  • Conduct patient visits, including new patient assessments and follow-ups, adhering to scheduled time slots.
  • Engage with hospitalists and transitional care teams to ensure continuity of care for hospitalized patients.
  • Participate in clinical meetings and quality management programs to align with organizational goals.
  • Maintain accurate and concise documentation of patient interactions and care plans using electronic medical records.
  • Promote patient education and health literacy to empower individuals in managing their health effectively.

Job Requirements

  • Doctor of Medicine (MD) degree with active state medical license and DEA certification.
  • Board certification in Internal Medicine, Family Medicine, or Geriatrics, maintaining certification through CME and MOC activities.
  • Minimum of 1 year of experience in primary care, with 5 years preferred.
  • Proficiency in electronic medical records and Microsoft Office Suite for documentation and communication.
  • Strong commitment to patient-centered care and value-based healthcare delivery models.
  • Ability to lead and collaborate with multidisciplinary teams to achieve clinical and organizational goals.
  • Fluency in English, with Spanish language skills preferred for specific locations.
  • Willingness to participate in training programs and travel locally or nationally as required.
