Key Responsibilities & Duties
- Diagnose and treat acute and chronic illnesses in older adult patients, ensuring comprehensive care management.
- Develop and implement individualized care plans, coordinating with specialists and support teams as needed.
- Lead care teams, including medical assistants and care coordinators, to deliver efficient and effective patient care.
- Conduct patient visits, including new patient assessments and follow-ups, adhering to scheduled time slots.
- Engage with hospitalists and transitional care teams to ensure continuity of care for hospitalized patients.
- Participate in clinical meetings and quality management programs to align with organizational goals.
- Maintain accurate and concise documentation of patient interactions and care plans using electronic medical records.
- Promote patient education and health literacy to empower individuals in managing their health effectively.
Job Requirements
- Doctor of Medicine (MD) degree with active state medical license and DEA certification.
- Board certification in Internal Medicine, Family Medicine, or Geriatrics, maintaining certification through CME and MOC activities.
- Minimum of 1 year of experience in primary care, with 5 years preferred.
- Proficiency in electronic medical records and Microsoft Office Suite for documentation and communication.
- Strong commitment to patient-centered care and value-based healthcare delivery models.
- Ability to lead and collaborate with multidisciplinary teams to achieve clinical and organizational goals.
- Fluency in English, with Spanish language skills preferred for specific locations.
- Willingness to participate in training programs and travel locally or nationally as required.
