Key Responsibilities & Duties

• Deliver primary care services to patients in long-term care settings.
• Diagnose and manage a variety of acute and chronic health conditions.
• Prescribe medications and oversee treatment plans in compliance with regulations.
• Collaborate with physicians and interdisciplinary teams for optimal patient outcomes.
• Utilize EMR systems to maintain accurate patient records and care plans.
• Coordinate care for patients with advanced illnesses and multiple chronic conditions.
• Perform diagnostic testing and ensure timely follow-up on results.
• Engage in discussions with interdisciplinary teams to improve care strategies.

Job Requirements

• Doctorate of Nursing Practice (DNP) with an unrestricted NP license.
• Board Certification and DEA licensure are essential.
• Experience in adult, family, or geriatric specialties is preferred.
• Proficiency in electronic medical records (EMR) systems is required.
• Ability to manage complex health conditions and advanced illnesses effectively.
• Strong collaboration skills within interdisciplinary care teams are vital.
• Experience in long-term care settings is highly desirable.
• Psychiatric experience is considered an advantageous skill.
