Overview
- Deliver high-quality primary and preventive care to patients in skilled nursing facilities, focusing on chronic condition management and acute care interventions.

- Prescribe medications, including controlled substances, in compliance with state regulations and clinical guidelines.
- Order and interpret diagnostic tests, ensuring timely follow-up and effective treatment adjustments.

- Utilize evidence-based practices to enhance patient outcomes and support facility quality improvement initiatives.

Key Responsibilities & Duties
- Conduct thorough assessments of patients in long-term care settings to identify health concerns and develop treatment plans.
- Implement individualized care strategies in collaboration with physicians and other healthcare professionals.
- Monitor and adjust care plans for patients with complex medical conditions, ensuring optimal health outcomes.
- Educate patients and families on disease management, preventive care, and treatment options.
- Maintain accurate and up-to-date documentation using electronic medical records (EMR) systems.
- Participate in interdisciplinary team meetings to discuss patient progress and coordinate care.

Job Requirements
- Doctorate of Nursing Practice (DNP) with active board certification in relevant specialties.
- Current, unrestricted Nurse Practitioner license in the state of practice.
- DEA licensure for prescribing controlled substances.
- Minimum of 1 year clinical experience, with 3 years preferred, in adult, family, or geriatric care.
- Proficiency in managing patients with advanced illnesses and chronic conditions.
- Experience in long-term care settings and interdisciplinary team collaboration.
- Knowledge of electronic medical records (EMR) systems.
- Psychiatric experience is advantageous but not mandatory.
