Job Description

Overview

  • Deliver high-quality primary and preventive care to patients in skilled nursing facilities, focusing on chronic condition management and acute care interventions.
  • Collaborate with interdisciplinary teams to develop comprehensive care plans tailored to individual patient needs.
  • Prescribe medications, including controlled substances, in compliance with state regulations and clinical guidelines.
  • Order and interpret diagnostic tests, ensuring timely follow-up and effective treatment adjustments.
  • Engage in advanced care planning discussions to align treatment goals with patient preferences.
  • Utilize evidence-based practices to enhance patient outcomes and support facility quality improvement initiatives.
  • Communicate effectively with nursing staff and families to ensure clarity in care plans and patient progress.
  • Contribute to the development of protocols and strategies to improve CMS ratings and patient satisfaction.

Key Responsibilities & Duties

  • Conduct thorough assessments of patients in long-term care settings to identify health concerns and develop treatment plans.
  • Implement individualized care strategies in collaboration with physicians and other healthcare professionals.
  • Monitor and adjust care plans for patients with complex medical conditions, ensuring optimal health outcomes.
  • Educate patients and families on disease management, preventive care, and treatment options.
  • Maintain accurate and up-to-date documentation using electronic medical records (EMR) systems.
  • Participate in interdisciplinary team meetings to discuss patient progress and coordinate care.
  • Ensure compliance with healthcare regulations, standards, and ethical practices.
  • Contribute to research and quality improvement initiatives within the facility.

Job Requirements

  • Doctorate of Nursing Practice (DNP) with active board certification in relevant specialties.
  • Current, unrestricted Nurse Practitioner license in the state of practice.
  • DEA licensure for prescribing controlled substances.
  • Minimum of 1 year clinical experience, with 3 years preferred, in adult, family, or geriatric care.
  • Proficiency in managing patients with advanced illnesses and chronic conditions.
  • Experience in long-term care settings and interdisciplinary team collaboration.
  • Knowledge of electronic medical records (EMR) systems.
  • Psychiatric experience is advantageous but not mandatory.
