Job Description

Overview

  • Deliver specialized wound care services to patients in skilled nursing facilities, ensuring optimal healing and patient satisfaction.
  • Prescribe medications, including controlled substances, adhering to state regulations and best practices.
  • Collaborate with primary care physicians and interdisciplinary teams to provide comprehensive patient care.
  • Utilize advanced wound care products and techniques to manage diverse wound types effectively.
  • Diagnose and treat various wound conditions, including diabetic, arterial, and venous wounds.
  • Communicate effectively with nursing home staff regarding patient conditions and care plans.
  • Maintain accurate and detailed records using electronic medical record systems.
  • Participate in interdisciplinary care teams to coordinate and optimize patient care outcomes.

Key Responsibilities & Duties

  • Conduct thorough assessments and provide expert wound care for long-term care patients.
  • Order and interpret diagnostic tests, ensuring timely follow-up and treatment adjustments.
  • Educate patients and caregivers on effective wound care management and prevention strategies.
  • Utilize wound vacs and pressure-relieving devices to enhance patient recovery.
  • Stage pressure injuries accurately following established clinical guidelines.
  • Prescribe advanced wound care products tailored to individual patient needs.
  • Collaborate with interdisciplinary teams to develop and implement effective care plans.
  • Ensure compliance with healthcare regulations and standards in all aspects of care delivery.

Job Requirements

  • Doctorate of Nursing Practice (DNP) with specialization in adult, family, or geriatric care.
  • Current, unrestricted Nurse Practitioner license in the state of Ohio.
  • Minimum of 1 year of experience in wound care; 3 years preferred.
  • Proficiency in diagnosing and treating diabetic, arterial, and venous wounds.
  • Familiarity with advanced wound care products and techniques.
  • Comfortable using electronic medical record systems for documentation and communication.
  • Wound care certification strongly preferred or willingness to obtain certification.
  • Ability to work collaboratively within an interdisciplinary care team environment.
