Job Description

Overview

  • Provide health education and screenings to members, promoting wellness and healthier lifestyle behaviors.
  • Administer vaccinations, conduct health assessments, and monitor chronic conditions.
  • Participate in health fairs and outreach events within the local community.
  • Educate members on health benefits and refer them to appropriate resources.
  • Collaborate with physicians and specialists regarding member health status.
  • Record clinical data for statistical analysis and reporting purposes.
  • Perform additional duties and projects as assigned by management.

Key Responsibilities & Duties

  • Screen and monitor members for acute and chronic conditions using various health metrics.
  • Administer tests and vaccinations, ensuring proper documentation and follow-up.
  • Educate members on healthier lifestyle choices and chronic condition management.
  • Participate in community outreach events to promote wellness initiatives.
  • Counsel members on available health benefits and connect them with relevant services.
  • Collaborate with healthcare professionals to address member health concerns.
  • Maintain accurate records of clinical data and member interactions.
  • Travel to various sites within the New York Metropolitan area as required.

Job Requirements

  • Valid New York State Registered Nurse (RN) license; Bachelor’s degree preferred.
  • Minimum two years of nursing experience in ambulatory, community, or medical/surgical care.
  • Proficiency in clinical assessments, patient education, and motivational interviewing.
  • Intermediate computer skills, including Microsoft Office applications.
  • Bilingual in Spanish preferred; excellent communication skills required.
  • Ability to travel within the New York Metropolitan area; valid driver’s license preferred.
  • Flexibility to work evenings and weekends as needed.
