Key Responsibilities & Duties
- Conduct assessments to identify residents' psychosocial needs and develop appropriate interventions.
- Facilitate communication between residents, families, and care teams to ensure shared understanding of care plans.
- Coordinate discharge planning and follow-up services for residents transitioning to other care settings.
- Provide emotional support and counseling to residents and families during challenging times.
- Maintain accurate and timely documentation of social work activities and resident progress.
- Collaborate with interdisciplinary teams to enhance service delivery and resident satisfaction.

Job Requirements
- Master’s degree in Social Work (MSW) required; LMSW certification preferred.
- Minimum of 1 year experience in social work; 2 years preferred.
- Experience in geriatrics, long-term care, or healthcare settings is advantageous.
- Proficiency in discharge planning and case management for aging populations.
- Strong interpersonal and communication skills to interact effectively with residents and families.
