Job Description

Overview

  • Provide comprehensive social work services to residents and families, ensuring optimal quality of life and care.
  • Collaborate with a multidisciplinary team to address residents' needs and develop personalized care plans.
  • Assist in managing departmental operations and contribute to the improvement of care services.
  • Support residents in discharge planning, ensuring smooth transitions and continuity of care.
  • Engage in case management activities, providing guidance and advocacy for residents and families.
  • Maintain compliance with state and federal regulations related to social work practices.
  • Participate in holiday and weekend rotation schedules to ensure consistent service delivery.
  • Utilize expertise in geriatrics to address the unique needs of aging populations.

Key Responsibilities & Duties

  • Conduct assessments to identify residents' psychosocial needs and develop appropriate interventions.
  • Facilitate communication between residents, families, and care teams to ensure shared understanding of care plans.
  • Coordinate discharge planning and follow-up services for residents transitioning to other care settings.
  • Provide emotional support and counseling to residents and families during challenging times.
  • Maintain accurate and timely documentation of social work activities and resident progress.
  • Collaborate with interdisciplinary teams to enhance service delivery and resident satisfaction.
  • Participate in staff meetings and training sessions to stay updated on best practices.
  • Assist in the development and implementation of departmental policies and procedures.

Job Requirements

  • Master’s degree in Social Work (MSW) required; LMSW certification preferred.
  • Minimum of 1 year experience in social work; 2 years preferred.
  • Experience in geriatrics, long-term care, or healthcare settings is advantageous.
  • Proficiency in discharge planning and case management for aging populations.
  • Strong interpersonal and communication skills to interact effectively with residents and families.
  • Ability to work on-site and participate in holiday and weekend rotations.
  • Commitment to providing excellent customer service and advocacy for residents.
  • Knowledge of state and federal regulations governing social work practices.
