Job Description

Overview

  • Assist in managing department operations to ensure high-quality care for residents and their families.
  • Collaborate with a multidisciplinary care team to enhance residents' quality of life.
  • Provide support in discharge planning and case management assessments.
  • Utilize Point Click Care (PCC) software for efficient case management.
  • Work onsite to deliver personalized services to the aging population.
  • Engage in training to improve customer service for residents and families.
  • Support the department's goal of providing exceptional long-term care services.
  • Contribute to the development of innovative care strategies.

Key Responsibilities & Duties

  • Coordinate resident services to ensure smooth departmental operations.
  • Guide the care team in implementing effective care plans.
  • Assist in discharge planning and case management for residents.
  • Provide training and support to staff in customer service delivery.
  • Utilize PCC software for documentation and care coordination.
  • Assess residents' needs to develop personalized care strategies.
  • Collaborate with families to address concerns and improve satisfaction.
  • Ensure compliance with healthcare regulations and standards.

Job Requirements

  • Master’s degree in Social Work (MSW) required; LMSW certification preferred.
  • Experience in geriatrics, long-term care, or healthcare preferred.
  • Proficiency in Point Click Care (PCC) software is advantageous.
  • Strong skills in discharge planning and case management assessments.
  • Ability to provide excellent customer service to residents and families.
