Overview

Key Responsibilities & Duties
- Coordinate and oversee resident services to ensure high-quality care and satisfaction.
- Facilitate interdisciplinary team meetings to discuss resident care plans and progress.
- Conduct assessments to identify resident needs and develop personalized care strategies.
- Manage discharge planning processes, collaborating with families and external providers.
- Utilize PCC systems for documentation and communication within the care team.
- Provide guidance and support to residents and families, addressing concerns and questions.

Job Requirements
- Master’s degree in Social Work (MSW) required; LMSW certification preferred.
- Experience in aging, long-term care, or healthcare preferred.
- Proficiency in discharge planning and case management essential.
- Familiarity with Point Click Care (PCC) systems preferred.
- Strong customer service skills for interactions with residents and families.
