Overview

- Educate patients and families on home exercises and discharge plans.
- Participate in quality improvement initiatives and departmental meetings.

Key Responsibilities & Duties
- Conduct comprehensive functional assessments and design individualized care plans.
- Perform therapy treatments as prescribed by physicians, adhering to clinical guidelines.
- Supervise and train support staff in therapeutic procedures and patient care.
- Communicate effectively with patients, families, and healthcare professionals regarding progress and discharge plans.
- Document treatment plans, progress notes, and discharge summaries accurately.
- Educate patients and caregivers on therapeutic exercises and safety precautions.
- Participate in marketing and community outreach activities as directed.
- Ensure compliance with regulatory and professional standards.
- Provide clinical supervision to students and assist in their professional development.
Job Requirements
- Master of Science in Physical Therapy or equivalent degree required.
- Current California Physical Therapist License and AHA BLS certification mandatory.
- Minimum of 1 year of experience preferred; new graduates considered.
- Proficiency in bilingual communication (English/Spanish) preferred.
- Ability to lift objects up to 35 lbs and perform physical tasks.
- Strong interpersonal skills and adaptability to diverse patient needs.
- Experience with pediatric, adolescent, adult, and geriatric patients.
