Job Description

Overview

  • Support home care operations by managing patient intake processes efficiently and accurately.
  • Serve as a key contact for patients and families, ensuring smooth coordination and compliance.
  • Utilize bilingual Spanish and English skills to communicate effectively with diverse stakeholders.
  • Work in a fast-paced office environment within a home care agency setting.
  • Collaborate with clinical, administrative, and billing teams to optimize patient care processes.
  • Ensure HIPAA compliance and confidentiality in all interactions and documentation.
  • Contribute to the agency's success by tracking and reporting intake metrics effectively.
  • Provide general administrative support to enhance operational efficiency.

Key Responsibilities & Duties

  • Manage the full intake process for new home care patients, ensuring accuracy and completeness.
  • Collect and verify patient demographics and insurance information promptly.
  • Receive and process referrals from hospitals, physicians, and community sources.
  • Conduct intake interviews and gather necessary documentation efficiently.
  • Verify insurance eligibility and coverage to facilitate patient care.
  • Follow up on incomplete referrals and pending documents proactively.
  • Track and report intake metrics, including conversion rates and referral sources.
  • Handle high call volumes professionally and efficiently, resolving issues as needed.

Job Requirements

  • Associate of Science (AS) degree or equivalent educational background.
  • Minimum of 1 year of direct home care intake experience, with 3 years preferred.
  • Fluency in English and Spanish, both spoken and written, is required.
  • Experience with HHA Exchange and familiarity with ePaces or Zoho systems preferred.
  • Knowledge of Medicare and Medicaid processes is advantageous.
  • Proficiency in Microsoft Office and standard office systems is essential.
  • Strong data entry skills and attention to detail are critical for success.
  • Ability to multitask and work independently in a fast-paced environment.
