Key Responsibilities & Duties
- Manage the full intake process for new home care patients, ensuring accuracy and completeness.
- Collect and verify patient demographics and insurance information promptly.
- Receive and process referrals from hospitals, physicians, and community sources.
- Conduct intake interviews and gather necessary documentation efficiently.
- Verify insurance eligibility and coverage to facilitate patient care.
- Follow up on incomplete referrals and pending documents proactively.
- Track and report intake metrics, including conversion rates and referral sources.
- Handle high call volumes professionally and efficiently, resolving issues as needed.
Job Requirements
- Associate of Science (AS) degree or equivalent educational background.
- Minimum of 1 year of direct home care intake experience, with 3 years preferred.
- Fluency in English and Spanish, both spoken and written, is required.
- Experience with HHA Exchange and familiarity with ePaces or Zoho systems preferred.
- Knowledge of Medicare and Medicaid processes is advantageous.
- Proficiency in Microsoft Office and standard office systems is essential.
- Strong data entry skills and attention to detail are critical for success.
- Ability to multitask and work independently in a fast-paced environment.
