Overview
- Coordinate and oversee in-home care services ensuring compliance with healthcare standards and high-quality patient care.
- Lead a team of home care coordinators, providing guidance, training, and professional development opportunities.
- Address escalations involving patients, families, and healthcare providers to ensure service excellence.
- Promote continuous improvement in service delivery through process enhancements and quality initiatives.
- Collaborate with internal and external stakeholders to support seamless care transitions and patient safety.
- Ensure caregiver compliance with certifications, training, and regulatory requirements prior to placement.
- Monitor team productivity and adherence to processes, implementing workflow enhancements as needed.
- Prepare and present department updates, reports, and metrics to management.
Key Responsibilities & Duties
- Supervise daily operations of the coordination department, ensuring timely caregiver scheduling aligned with client needs.
- Provide performance feedback, training, and mentorship to coordinators for professional growth.
- Oversee case assignments, urgent coverage needs, and crisis interventions to minimize disruptions.
- Collaborate with Intake, Compliance, HR, and Field Operations for seamless onboarding and care transitions.
- Audit schedule entries and caregiver records to ensure data accuracy and completeness.
- Resolve scheduling conflicts, caregiver concerns, and client service issues promptly and professionally.
- Monitor team adherence to processes and key performance indicators, identifying improvement opportunities.
- Foster a collaborative work environment, promoting communication and accountability within the team.
Job Requirements
- Bachelor of Science (BS) degree or equivalent educational background.
- Minimum of 3 years of experience in home care coordination or healthcare scheduling.
- Preferred 5 years of experience, including supervisory roles in healthcare settings.
- Proficiency in scheduling software such as HHAeXchange or similar platforms.
- Strong knowledge of New York State DOH regulations and Medicaid/MLTC processes.
- Excellent organizational, time management, and leadership skills.
- Effective problem-solving and conflict resolution abilities.
- Bilingual proficiency in English and Spanish, Russian, or Cantonese is a plus.
