Overview
- Lead clinical operations for a residential treatment facility serving youth aged 12–18 with complex mental health diagnoses.
- Collaborate with interdisciplinary teams to ensure effective therapeutic and educational support for residents.
- Oversee transitions and discharge planning, ensuring successful reintegration into community settings.
- Provide clinical supervision to transitional and family support teams, enhancing rehabilitative and clinical competencies.
- Develop and implement permanency programs aimed at reducing length of stay and minimizing re-referrals.
- Ensure compliance with all relevant regulations and guidelines, maintaining high standards of care.
- Support communication and coordination between residents’ families, case managers, and schools.
- Contribute to the mission of providing comprehensive care tailored to individual needs.
Key Responsibilities & Duties
- Supervise and evaluate the performance of transitional coordinators and family support specialists.
- Coordinate with community-based programs to ensure efficient support for families and youth.
- Participate in clinical and treatment meetings, providing updates and insights on progress.
- Develop progressive home time plans and behavior management techniques for successful reunification.
- Maintain collaborative relationships with local resources and referral systems.
- Ensure proper utilization of transitional funds following established policies.
- Support compliance with documentation requirements through chart reviews and community visits.
- Serve as a critical member of the management team, contributing to strategic planning and implementation.
Job Requirements
- Master’s degree in Social Work or a related human services field.
- Minimum of 2 years of experience providing direct services to children with severe emotional disturbances.
- At least 2 years of supervisory or managerial experience in the social service sector.
- LCSW certification required.
- Experience with residential settings and working with youth with mental health challenges.
- Proficiency in electronic health records and familiarity with ACS, foster care, and housing systems.
- Ability to conduct home visits and navigate systems impacting youth and families.
- Strong interpersonal skills, adaptability, and dedication to supporting youth and families.
