Overview
- Lead and manage a team of nursing professionals within a hospice care setting.
- Ensure adherence to hospice and palliative care standards and organizational policies.
- Provide mentorship and guidance to nursing staff, promoting excellence in patient care.
- Collaborate with interdisciplinary teams to achieve patient and family care goals.
- Monitor and evaluate nursing documentation for compliance with regulations.
- Participate in community outreach and presentations to promote hospice services.
- Support the Associate Director in scheduling and staffing decisions.
- Maintain availability for follow-up on incidents beyond standard working hours.
Key Responsibilities & Duties
- Supervise and mentor nursing staff to ensure high-quality hospice care delivery.
- Coordinate staff assignments in line with organizational and regulatory standards.
- Empower RN Case Managers in pain and symptom management practices.
- Evaluate nursing documentation for accuracy and compliance with policies.
- Conduct community presentations to enhance awareness of hospice services.
- Collaborate with interdisciplinary teams to meet patient care objectives.
- Ensure availability for incident follow-ups outside regular working hours.
- Support the Associate Director in operational and strategic planning.
Job Requirements
- Bachelor of Science in Nursing (BSN) from an accredited institution.
- Minimum of five years of hospice nursing experience.
- Supervisory or leadership experience in a healthcare setting preferred.
- New York State RN licensure required.
- Certification in Hospice and Palliative Care (CHPN) preferred or obtainable within one year.
- Proficiency in Word, Outlook, and Windows applications.
- Strong ability to supervise and mentor nursing staff effectively.
- Experience in triage, on-call, or after-hours care is essential.
