Job Description

Overview

  • Lead and manage a team of nursing professionals within a hospice care setting.
  • Ensure adherence to hospice and palliative care standards and organizational policies.
  • Provide mentorship and guidance to nursing staff, promoting excellence in patient care.
  • Collaborate with interdisciplinary teams to achieve patient and family care goals.
  • Monitor and evaluate nursing documentation for compliance with regulations.
  • Participate in community outreach and presentations to promote hospice services.
  • Support the Associate Director in scheduling and staffing decisions.
  • Maintain availability for follow-up on incidents beyond standard working hours.

Key Responsibilities & Duties

  • Supervise and mentor nursing staff to ensure high-quality hospice care delivery.
  • Coordinate staff assignments in line with organizational and regulatory standards.
  • Empower RN Case Managers in pain and symptom management practices.
  • Evaluate nursing documentation for accuracy and compliance with policies.
  • Conduct community presentations to enhance awareness of hospice services.
  • Collaborate with interdisciplinary teams to meet patient care objectives.
  • Ensure availability for incident follow-ups outside regular working hours.
  • Support the Associate Director in operational and strategic planning.

Job Requirements

  • Bachelor of Science in Nursing (BSN) from an accredited institution.
  • Minimum of five years of hospice nursing experience.
  • Supervisory or leadership experience in a healthcare setting preferred.
  • New York State RN licensure required.
  • Certification in Hospice and Palliative Care (CHPN) preferred or obtainable within one year.
  • Proficiency in Word, Outlook, and Windows applications.
  • Strong ability to supervise and mentor nursing staff effectively.
  • Experience in triage, on-call, or after-hours care is essential.
