Job Description

Overview

  • Provide compassionate care to hospice patients, ensuring their physical and psychosocial needs are met during end-of-life stages.
  • Develop and implement personalized nursing care plans based on thorough assessments and patient requirements.
  • Collaborate with interdisciplinary teams to deliver holistic care and support to patients and families.
  • Supervise and train home health aides, homemakers, and personal care workers to ensure quality care delivery.
  • Request and manage physician orders for adjustments in patient care plans as needed.
  • Utilize clinical expertise to evaluate patient care and implement hands-on interventions effectively.
  • Ensure accurate clinical data input and documentation to maintain compliance and quality standards.
  • Work on-site in a dynamic healthcare environment, providing direct patient care and support.

Key Responsibilities & Duties

  • Conduct physical and psychosocial assessments for hospice patients to determine care needs.
  • Develop and implement nursing care plans tailored to individual patient requirements.
  • Supervise and evaluate the performance of home health aides and personal care workers.
  • Collaborate with physicians to obtain and manage written orders for patient care adjustments.
  • Provide education and support to patients and families regarding hospice care and processes.
  • Participate in interdisciplinary team meetings to ensure comprehensive care planning.
  • Maintain accurate and timely documentation of patient care activities and clinical data.
  • Ensure adherence to hospice care standards and regulations in all activities.

Job Requirements

  • Bachelor of Science in Nursing (BSN) from an accredited institution.
  • Minimum of 2 years of clinical experience in a healthcare setting.
  • Preferred experience in hospice care; home care or acute care experience considered.
  • Proficiency in clinical data input and documentation systems.
  • Strong communication, teaching, and listening skills for patient and team interactions.
  • Ability to work effectively within an interdisciplinary team environment.
  • Bilingual proficiency in Chinese is highly desirable.
