Overview
- Provide compassionate care to hospice patients, ensuring their physical and psychosocial needs are met during end-of-life stages.
- Develop and implement personalized nursing care plans based on thorough assessments and patient requirements.
- Collaborate with interdisciplinary teams to deliver holistic care and support to patients and families.
- Supervise and train home health aides, homemakers, and personal care workers to ensure quality care delivery.
- Request and manage physician orders for adjustments in patient care plans as needed.
- Utilize clinical expertise to evaluate patient care and implement hands-on interventions effectively.
- Ensure accurate clinical data input and documentation to maintain compliance and quality standards.

Key Responsibilities & Duties
- Conduct physical and psychosocial assessments for hospice patients to determine care needs.
- Develop and implement nursing care plans tailored to individual patient requirements.
- Supervise and evaluate the performance of home health aides and personal care workers.
- Collaborate with physicians to obtain and manage written orders for patient care adjustments.
- Provide education and support to patients and families regarding hospice care and processes.
- Participate in interdisciplinary team meetings to ensure comprehensive care planning.
- Maintain accurate and timely documentation of patient care activities and clinical data.
- Ensure adherence to hospice care standards and regulations in all activities.
Job Requirements
- Bachelor of Science in Nursing (BSN) from an accredited institution.
- Minimum of 2 years of clinical experience in a healthcare setting.
- Preferred experience in hospice care; home care or acute care experience considered.
- Proficiency in clinical data input and documentation systems.
- Strong communication, teaching, and listening skills for patient and team interactions.
- Ability to work effectively within an interdisciplinary team environment.
- Bilingual proficiency in Chinese is highly desirable.
