Job Description

Overview

  • Provide in-home therapy services to improve communication, cognitive-communication, and swallowing functions for patients.
  • Develop and implement individualized, goal-oriented treatment programs tailored to patient needs.
  • Ensure compliance with physician orders, organizational policies, and regulatory standards.
  • Collaborate with interdisciplinary team members to coordinate care and referrals effectively.
  • Educate patients and caregivers on communication strategies and home programs for continued improvement.
  • Maintain accurate and timely documentation of evaluations, progress notes, and discharge summaries.
  • Monitor patient progress and adjust treatment plans to achieve optimal outcomes.
  • Participate in discharge planning to ensure seamless transitions for patients.
  • Adhere to professional standards and organizational competencies in all activities.

Key Responsibilities & Duties

  • Conduct comprehensive patient evaluations in home settings within required timeframes.
  • Develop measurable short- and long-term goals in individualized treatment plans.
  • Deliver skilled speech-language pathology services aligned with physician’s care plans.
  • Maintain precise and complete EMR documentation to support skilled care.
  • Adjust treatment plans as needed based on patient progress and feedback.
  • Educate patients and caregivers on effective communication strategies and home programs.
  • Collaborate with physicians and team members to ensure coordinated care delivery.
  • Participate actively in discharge planning to support patient transitions.
  • Maintain productivity standards while demonstrating flexibility in duties.

Job Requirements

  • Master’s Degree in Speech-Language Pathology is required for this position.
  • Active Speech Language Pathologist License (NJ) and AHA Basic Life Support Certification.
  • Minimum of 6 months of experience in Speech Language Pathology.
  • Proficiency in using computer systems and electronic medical records (EMR).
  • Ability to lift, push, and pull up to 50 lbs and stand for extended periods.
  • Strong organizational skills and adherence to professional standards.
  • Experience in home health settings is preferred for this role.
  • Excellent communication and interpersonal skills for patient and team interactions.
