Overview
- Provide in-home therapy services to improve communication, cognitive-communication, and swallowing functions for patients.
- Develop and implement individualized, goal-oriented treatment programs tailored to patient needs.
- Ensure compliance with physician orders, organizational policies, and regulatory standards.
- Collaborate with interdisciplinary team members to coordinate care and referrals effectively.
- Educate patients and caregivers on communication strategies and home programs for continued improvement.
- Maintain accurate and timely documentation of evaluations, progress notes, and discharge summaries.
- Monitor patient progress and adjust treatment plans to achieve optimal outcomes.
- Participate in discharge planning to ensure seamless transitions for patients.
- Adhere to professional standards and organizational competencies in all activities.
Key Responsibilities & Duties
- Conduct comprehensive patient evaluations in home settings within required timeframes.
- Develop measurable short- and long-term goals in individualized treatment plans.
- Deliver skilled speech-language pathology services aligned with physician’s care plans.
- Maintain precise and complete EMR documentation to support skilled care.
- Adjust treatment plans as needed based on patient progress and feedback.
- Educate patients and caregivers on effective communication strategies and home programs.
- Collaborate with physicians and team members to ensure coordinated care delivery.
- Participate actively in discharge planning to support patient transitions.
- Maintain productivity standards while demonstrating flexibility in duties.
Job Requirements
- Master’s Degree in Speech-Language Pathology is required for this position.
- Active Speech Language Pathologist License (NJ) and AHA Basic Life Support Certification.
- Minimum of 6 months of experience in Speech Language Pathology.
- Proficiency in using computer systems and electronic medical records (EMR).
- Ability to lift, push, and pull up to 50 lbs and stand for extended periods.
- Strong organizational skills and adherence to professional standards.
- Experience in home health settings is preferred for this role.
- Excellent communication and interpersonal skills for patient and team interactions.
