Job Description

Overview

  • Provide integrated behavioral health services in a primary care setting, utilizing brief, solution-focused interventions to enhance patient wellbeing.
  • Collaborate with medical and dental teams to address psychosocial stressors and improve patient adherence to treatment plans.
  • Support patients experiencing depression and anxiety through evidence-based interventions and innovative care delivery methods.
  • Refer patients to external providers for ongoing therapy, ensuring comprehensive care coordination.
  • Contribute to the development of educational programs addressing diverse population needs.
  • Maintain compliance with Joint Commission and NCQA accreditation standards and organizational protocols.
  • Document care provided, adhering to confidentiality and patient privacy regulations.
  • Engage in performance improvement processes to enhance service quality and patient outcomes.

Key Responsibilities & Duties

  • Conduct comprehensive behavioral health assessments and develop differential diagnoses and intervention plans.
  • Collaborate with primary care teams to provide integrated behavioral health services to patients.
  • Utilize evidence-based behavioral health assessment and treatment modalities in patient care.
  • Facilitate ethical decision-making processes and act as a consultant when requested.
  • Refer patients for further care and services within the organization or community as needed.
  • Apply innovative delivery methods, including brief interventions and telehealth, to enhance patient outcomes.
  • Engage families in patient care plans to ensure comprehensive support.
  • Report to the Director of Integrated Behavioral Health and contribute to organizational goals.

Job Requirements

  • Master of Social Work degree and clinical licensure as LCSW or LPC-MHSP in Tennessee.
  • Minimum of 6 months of experience required; 2 years preferred.
  • Proficiency in Spanish, Arabic, or Kurdish is preferred.
  • Experience with trauma, co-occurring disorders, and evidence-based therapies preferred.
  • Ability to communicate effectively with patients, families, and care teams.
  • Flexibility and motivation to work in a primary care setting.
  • Prior experience in an integrated healthcare setting is preferred.
  • Competence in innovative care delivery methods, including brief interventions and telehealth.
