Overview

  • Provide compassionate hospice care during after-hours shifts, addressing urgent patient needs and ensuring seamless care delivery.
  • Coordinate with triage teams and physicians to implement effective care plans and manage patient symptoms.
  • Educate patients and caregivers on medication management and care strategies to enhance comfort and quality of life.
  • Offer emotional support to families during challenging moments, fostering a compassionate environment.
  • Ensure compliance with professional nursing standards and state regulations for hospice care.
  • Engage in both office and field settings, utilizing resources effectively for patient visits.
  • Collaborate with interdisciplinary teams to enhance patient and family experiences in hospice care.

Key Responsibilities & Duties

  • Conduct patient visits to address urgent care needs and implement care plans effectively.
  • Coordinate with on-call teams to ensure timely and safe delivery of hospice care services.
  • Educate caregivers on care plan adjustments, medication protocols, and safety measures.
  • Provide emotional support and guidance to families during patient deaths and transitions.
  • Report patient visit outcomes to the primary hospice team for follow-up care planning.
  • Ensure adherence to hospice care guidelines and professional nursing standards.
  • Utilize field and office resources efficiently to deliver high-quality patient care.
  • Collaborate with physicians and care teams to address patient needs promptly and effectively.

Job Requirements

  • Bachelor’s degree in Nursing from an accredited institution is required.
  • Minimum of two years of experience in Medical-Surgical, Community Health, Hospice, or Oncology nursing preferred.
  • Active RN licensure in New York State and a valid driver’s license preferred.
  • Commitment to obtaining CHPN certification within one year of hire.
  • Proficiency in Word, Excel, and Windows applications is essential.
  • Strong communication skills to coordinate with hospice care teams and physicians effectively.
  • Ability to provide compassionate care and emotional support to patients and families.
  • Flexibility to work varied shifts, including weekends, as required.
