Key Responsibilities & Duties
- Conduct patient visits to address urgent care needs and implement care plans effectively.
- Coordinate with on-call teams to ensure timely and safe delivery of hospice care services.

- Provide emotional support and guidance to families during patient deaths and transitions.
- Report patient visit outcomes to the primary hospice team for follow-up care planning.

Job Requirements
- Bachelor’s degree in Nursing from an accredited institution is required.
- Minimum of two years of experience in Medical-Surgical, Community Health, Hospice, or Oncology nursing preferred.
- Active RN licensure in New York State and a valid driver’s license preferred.
- Commitment to obtaining CHPN certification within one year of hire.
- Proficiency in Word, Excel, and Windows applications is essential.
- Strong communication skills to coordinate with hospice care teams and physicians effectively.
- Ability to provide compassionate care and emotional support to patients and families.
- Flexibility to work varied shifts, including weekends, as required.
