Job Description

Overview

  • Deliver personalized nutrition care to participants in a dynamic adult day program, supporting their health and wellness goals.
  • Collaborate with interdisciplinary teams to create comprehensive care plans tailored to individual dietary needs.
  • Provide education and guidance on nutrition to participants and their families, fostering informed dietary choices.
  • Ensure compliance with dietary prescriptions and regulatory guidelines, maintaining high standards of care.
  • Develop and implement nutrition programs that respect cultural and religious dietary preferences.
  • Assist in meal planning and preparation, ensuring nutritional adequacy and variety for participants.
  • Contribute to program improvement initiatives, enhancing the quality of nutrition services offered.
  • Document nutritional care plans and progress accurately, adhering to best practices and standards.

Key Responsibilities & Duties

  • Conduct comprehensive nutritional assessments for program participants, identifying individual needs and goals.
  • Participate in interdisciplinary meetings to develop and review care plans for participants.
  • Design and adjust therapeutic diets in alignment with physician prescriptions and participant health requirements.
  • Provide one-on-one dietary counseling to participants, promoting sustainable and beneficial nutrition practices.
  • Collaborate on menu development to accommodate diverse dietary needs and preferences.
  • Support audits and quality improvement efforts related to nutrition services and program outcomes.
  • Ensure timely and accurate documentation of nutritional interventions and participant progress.
  • Provide training and education to staff on nutrition-related topics to enhance program effectiveness.

Job Requirements

  • Registered Dietitian credential with experience in clinical or long-term care settings preferred.
  • Strong ability to assess nutritional needs and develop individualized care plans.
  • Effective communication skills for participant counseling and team collaboration.
  • Proficiency in documentation practices adhering to regulatory and professional standards.
  • Experience working with older adults in settings such as adult day programs or long-term care.
  • Ability to respect and integrate cultural and religious dietary preferences into care plans.
  • High School Diploma or GED required; degree in Nutrition or related field preferred.
  • Availability for on-site work during scheduled hours, Monday to Friday.
