Overview
- Provide primary and preventive care for patients in skilled nursing facilities.
- Manage residents experiencing acute changes in condition and chronic care needs.
- Prescribe medications, including controlled substances, adhering to state regulations.
- Collaborate with interdisciplinary care teams to ensure comprehensive patient care.
- Utilize electronic medical records (EMR) for patient documentation and management.
- Engage in advanced care planning and chronic disease management strategies.
- Communicate effectively with nursing home staff regarding patient care plans.
- Contribute to improving CMS ratings through outcome-driven resident care.
Key Responsibilities & Duties
- Conduct thorough assessments and diagnostics for long-term care patients.
- Develop and implement individualized care plans for residents.
- Coordinate with primary care physicians to optimize patient outcomes.
- Order and review diagnostic tests and follow up on treatment plans.
- Prescribe medications and monitor their effectiveness and compliance.

Job Requirements
- Master of Science in Nursing with a focus on adult, family, or geriatric care.
- Current, unrestricted Nurse Practitioner license in the state of practice.
- Board Certification and DEA licensure are mandatory.
- Experience in long-term care settings and managing chronic conditions.
- Proficiency in using electronic medical records (EMR) systems.
