Overview
- Lead the Care Management Department, ensuring effective coordination, case management, and discharge planning processes.
- Act as a liaison between patients, families, healthcare staff, and community agencies to ensure seamless care delivery.
- Implement and monitor clinical standards and best practices to maintain high-quality patient care.
- Oversee utilization management and pre-admission case management activities within the department.
- Collaborate with managed care representatives and skilled nursing facilities to optimize patient outcomes.
- Ensure compliance with regulatory requirements and organizational policies.
- Support staff development and training to enhance departmental capabilities.
- Contribute to the financial integrity of the organization through effective resource management.
Key Responsibilities & Duties
- Manage daily operations of the Care Management Department, including staff supervision and resource allocation.
- Develop and implement care coordination strategies to improve patient outcomes.
- Monitor and evaluate departmental performance against established benchmarks and standards.
- Facilitate communication and collaboration among multidisciplinary teams.
- Oversee case management processes to ensure effective discharge planning and utilization management.
- Provide guidance and support to staff in handling complex cases and situations.
- Maintain accurate documentation and reporting for compliance and quality assurance purposes.
- Identify opportunities for process improvement and implement solutions to enhance departmental efficiency.
Job Requirements
- Master's Degree in Social Work from an accredited institution.
- Washington Clinical Independent Social Worker License is required.
- Minimum of 6 years of experience in case management within an acute care setting.
- At least 2 years of managerial or director-level experience in a large hospital.
- Strong knowledge of care coordination, utilization management, and discharge planning processes.
- Proven ability to lead and develop teams in a healthcare environment.
- Excellent communication and interpersonal skills for collaboration with diverse stakeholders.
