Job Description

Overview

  • Lead the Care Management Department, ensuring effective coordination, case management, and discharge planning processes.
  • Act as a liaison between patients, families, healthcare staff, and community agencies to ensure seamless care delivery.
  • Implement and monitor clinical standards and best practices to maintain high-quality patient care.
  • Oversee utilization management and pre-admission case management activities within the department.
  • Collaborate with managed care representatives and skilled nursing facilities to optimize patient outcomes.
  • Ensure compliance with regulatory requirements and organizational policies.
  • Support staff development and training to enhance departmental capabilities.
  • Contribute to the financial integrity of the organization through effective resource management.

Key Responsibilities & Duties

  • Manage daily operations of the Care Management Department, including staff supervision and resource allocation.
  • Develop and implement care coordination strategies to improve patient outcomes.
  • Monitor and evaluate departmental performance against established benchmarks and standards.
  • Facilitate communication and collaboration among multidisciplinary teams.
  • Oversee case management processes to ensure effective discharge planning and utilization management.
  • Provide guidance and support to staff in handling complex cases and situations.
  • Maintain accurate documentation and reporting for compliance and quality assurance purposes.
  • Identify opportunities for process improvement and implement solutions to enhance departmental efficiency.

Job Requirements

  • Master's Degree in Social Work from an accredited institution.
  • Washington Clinical Independent Social Worker License is required.
  • Minimum of 6 years of experience in case management within an acute care setting.
  • At least 2 years of managerial or director-level experience in a large hospital.
  • Strong knowledge of care coordination, utilization management, and discharge planning processes.
  • Proven ability to lead and develop teams in a healthcare environment.
  • Excellent communication and interpersonal skills for collaboration with diverse stakeholders.
