Job Description

Overview

  • Manage high-tech and complex referrals in the Access Center for hospice admissions.
  • Monitor referral lists and coordinate timely admissions to the hospice program.
  • Collaborate with interdisciplinary teams to ensure safe and effective admissions.
  • Verify and process all written and verbal orders for evaluation prior to admission.
  • Work on-site covering areas within Brooklyn and the Bronx.
  • Engage in patient-focused care with a commitment to quality service.
  • Utilize strong communication and customer service skills to enhance patient experiences.
  • Maintain accurate documentation and utilize PC skills effectively.

Key Responsibilities & Duties

  • Coordinate and manage high-tech hospice referrals within the Access Center.
  • Ensure timely admission of referrals to the hospice program.
  • Collaborate with nurses, physicians, and social workers for safe admissions.
  • Verify and process evaluation orders before program admission.
  • Monitor referral lists and coordinate with Access Coordinators.
  • Provide excellent customer service and communication with patients and families.
  • Maintain accurate records and utilize technical skills effectively.
  • Participate in weekend shifts as required, ensuring consistent care delivery.

Job Requirements

  • Graduate from an accredited school of Nursing with an AAS degree; BSN preferred.
  • Active NY State RN License with relevant acute care or hospice experience.
  • Minimum of 2 years of experience; 6 years preferred.
  • Proficiency in PC skills and documentation management.
  • Strong written and oral communication skills.
  • Availability for on-site work covering Brooklyn and the Bronx areas.
  • Ability to work weekends as needed, ensuring consistent patient care.
  • Experience in ER, Critical Care, Oncology, or Long-Term Care is advantageous.
