Hospice Team Manager

Overview

• Lead and mentor a team of nursing professionals in a hybrid work environment.
• Ensure delivery of high-quality Hospice and Palliative Care services aligned with organizational goals.
• Collaborate with interdisciplinary teams to optimize patient care and family support.
• Provide guidance on pain management and symptom control for patients.
• Oversee and evaluate nursing documentation to ensure compliance with regulations.
• Participate in community outreach and presentations to promote hospice services.
• Support orientation and training for new staff members at designated locations.
• Work in a flexible schedule including rotating weekends as needed.

Key Responsibilities & Duties

• Supervise and mentor a team of 6–10 full-time and 2–3 fee-for-service staff.
• Coordinate staff assignments adhering to staffing ratios and organizational policies.
• Monitor quality and compliance of nursing documentation with state and federal regulations.
• Empower RN Case Managers to excel in patient care and symptom management.
• Participate in interdisciplinary team meetings and contribute to care planning.
• Facilitate community presentations to enhance awareness of hospice services.
• Support initial orientation and quarterly training sessions for staff.
• Ensure adherence to organizational policies and procedures in daily operations.

Job Requirements

• Bachelor of Science in Nursing (BSN) from an accredited institution.
• Minimum of five years of Hospice nursing experience required.
• Supervisory or leadership experience in a healthcare setting preferred.
• Active New York State RN licensure required.
• National Board Certification in Hospice and Palliative Care (CHPN) preferred or obtainable within one year.
• Proficiency in Word, Outlook, and Windows applications.
• Strong ability to supervise and mentor nursing staff effectively.
• Capability to work collaboratively within interdisciplinary teams.
