Overview
- Lead and mentor a team of nursing professionals in a hybrid work environment.
- Ensure delivery of high-quality Hospice and Palliative Care services aligned with organizational goals.
- Collaborate with interdisciplinary teams to optimize patient care and family support.
- Provide guidance on pain management and symptom control for patients.
- Oversee and evaluate nursing documentation to ensure compliance with regulations.
- Participate in community outreach and presentations to promote hospice services.
- Support orientation and training for new staff members at designated locations.
- Work in a flexible schedule including rotating weekends as needed.
Key Responsibilities & Duties
- Supervise and mentor a team of 6–10 full-time and 2–3 fee-for-service staff.
- Coordinate staff assignments adhering to staffing ratios and organizational policies.
- Monitor quality and compliance of nursing documentation with state and federal regulations.
- Empower RN Case Managers to excel in patient care and symptom management.
- Participate in interdisciplinary team meetings and contribute to care planning.
- Facilitate community presentations to enhance awareness of hospice services.
- Support initial orientation and quarterly training sessions for staff.
- Ensure adherence to organizational policies and procedures in daily operations.
Job Requirements
- Bachelor of Science in Nursing (BSN) from an accredited institution.
- Minimum of five years of Hospice nursing experience required.
- Supervisory or leadership experience in a healthcare setting preferred.
- Active New York State RN licensure required.
- National Board Certification in Hospice and Palliative Care (CHPN) preferred or obtainable within one year.
- Proficiency in Word, Outlook, and Windows applications.
- Strong ability to supervise and mentor nursing staff effectively.
- Capability to work collaboratively within interdisciplinary teams.
