Overview

Key Responsibilities & Duties
- Serve as the primary medical case manager for assigned clients, ensuring comprehensive care coordination.
- Conduct regular health assessments and wellness checks to monitor client conditions.
- Collaborate with physicians, pharmacies, and psychiatric providers to address client needs.
- Support clients in accessing community resources and managing daily living tasks.
- Maintain detailed and accurate documentation of client interactions and medication administration.

Job Requirements
- Active Licensed Practical Nurse (LPN) license is mandatory.
- Associate of Applied Science (AAS) degree in nursing or related field required.
- Minimum of 1 year of experience; 3 years preferred in relevant roles.
- Strong interest in behavioral health, supportive housing, and community-based care.
- Proficiency in communication, documentation, and organizational skills.
