Key Responsibilities & Duties

  • Diagnose and manage acute and chronic conditions, creating individualized care plans for patients.
  • Collaborate with interdisciplinary teams to ensure comprehensive and coordinated patient care.

  • Coordinate with specialists and other providers to ensure seamless transitions in patient care.

Job Requirements

  • Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree is required.
  • Board certification in Internal Medicine, Family Medicine, or Geriatrics is mandatory.
  • Active state medical license or eligibility to obtain one promptly is essential.
  • Basic Life Support (BLS) certification required within 90 days of employment.
  • Proficiency in electronic medical records and healthcare technologies is preferred.
  • Strong commitment to serving underserved communities and improving health outcomes.
  • Excellent communication and leadership skills to effectively manage and inspire care teams.
  • Ability to travel locally for patient care and occasionally for training or recruitment.
  • Fluency in English; additional language skills may be advantageous.
