Overview
- Provide exceptional care as a Family Nurse Practitioner specializing in Women's Health and Primary Care.
- Deliver comprehensive healthcare services to diverse populations with varied medical needs.
- Conduct preventive screenings, chronic disease management, and well-woman exams with a patient-centered approach.
- Collaborate with multidisciplinary teams to ensure optimal patient outcomes and holistic care.
- Engage in both clinic-based and in-home patient care to enhance accessibility.
- Utilize evidence-based practices to address primary care and gynecological concerns effectively.
- Support patients through various life stages, including pregnancy, menopause, and chronic condition management.
- Ensure accurate and timely documentation within the Electronic Medical Record (EMR) system.
Key Responsibilities & Duties
- Perform comprehensive patient assessments, including history and physical exams, to evaluate health risks.
- Provide contraceptive counseling and advanced procedures such as IUD insertions.
- Diagnose and treat sexually transmitted infections and deliver sexual health education.
- Develop personalized care plans for chronic conditions like diabetes and hypertension.
- Coordinate specialized care with prenatal and other medical specialists.
- Screen for social determinants of health and connect patients with supportive resources.
- Provide accessible care through Tele-Health platforms and remote consultations.
- Ensure compliance with state regulations and clinical practice standards.
Job Requirements
- Master of Science in Nursing with Family Nurse Practitioner certification.
- Active RN licensure and DEA registration in the state of practice.
- Minimum of 1 year of experience in Women's Health and Primary Care; 2 years preferred.
- Proficiency in phlebotomy, immunizations, and office-based medical procedures.
- Experience addressing healthcare barriers for underserved populations.
- Fluency in an additional language is highly desirable.
- Strong clinical judgment and ability to collaborate effectively within teams.
