Job Description

Overview

  • Provide exceptional care as a Family Nurse Practitioner specializing in Women's Health and Primary Care.
  • Deliver comprehensive healthcare services to diverse populations with varied medical needs.
  • Conduct preventive screenings, chronic disease management, and well-woman exams with a patient-centered approach.
  • Collaborate with multidisciplinary teams to ensure optimal patient outcomes and holistic care.
  • Engage in both clinic-based and in-home patient care to enhance accessibility.
  • Utilize evidence-based practices to address primary care and gynecological concerns effectively.
  • Support patients through various life stages, including pregnancy, menopause, and chronic condition management.
  • Ensure accurate and timely documentation within the Electronic Medical Record (EMR) system.

Key Responsibilities & Duties

  • Perform comprehensive patient assessments, including history and physical exams, to evaluate health risks.
  • Provide contraceptive counseling and advanced procedures such as IUD insertions.
  • Diagnose and treat sexually transmitted infections and deliver sexual health education.
  • Develop personalized care plans for chronic conditions like diabetes and hypertension.
  • Coordinate specialized care with prenatal and other medical specialists.
  • Screen for social determinants of health and connect patients with supportive resources.
  • Provide accessible care through Tele-Health platforms and remote consultations.
  • Ensure compliance with state regulations and clinical practice standards.

Job Requirements

  • Master of Science in Nursing with Family Nurse Practitioner certification.
  • Active RN licensure and DEA registration in the state of practice.
  • Minimum of 1 year of experience in Women's Health and Primary Care; 2 years preferred.
  • Proficiency in phlebotomy, immunizations, and office-based medical procedures.
  • Experience addressing healthcare barriers for underserved populations.
  • Fluency in an additional language is highly desirable.
  • Strong clinical judgment and ability to collaborate effectively within teams.
