Key Responsibilities & Duties

  • Establish and maintain relationships with healthcare professionals to promote hospice services.
  • Conduct outreach activities to raise awareness about hospice care offerings.
  • Support patients and families by explaining hospice services and facilitating enrollment.
  • Collaborate with clinical staff to ensure seamless service delivery.

Job Requirements

  • Associate degree or equivalent experience in a relevant field.

  • Reliable transportation and valid driver’s license required.
