Job Description

Overview

  • Deliver exceptional nursing care and case management to individuals with developmental disabilities, ensuring compliance with healthcare standards and regulations.
  • Collaborate with interdisciplinary teams to design and implement personalized care plans tailored to individual needs.
  • Provide guidance and training to healthcare staff, fostering a culture of excellence and continuous improvement.
  • Monitor health outcomes and coordinate timely interventions to optimize patient well-being.
  • Ensure accurate documentation and management of medical records and care plans.
  • Participate in the development and implementation of organizational policies and compliance measures.
  • Maintain a proactive approach to emergency preparedness and on-call support.

Key Responsibilities & Duties

  • Conduct comprehensive assessments and develop individualized care plans for program participants.
  • Supervise and mentor healthcare staff, ensuring adherence to best practices and regulatory guidelines.
  • Coordinate health services, including medical appointments, treatments, and follow-ups.
  • Provide training on health and hygiene, communicable disease control, and client-specific care.
  • Monitor and evaluate health status, laboratory results, and treatment effectiveness.
  • Ensure accurate and timely documentation of medical records and training materials.
  • Participate in interdisciplinary team meetings to align care strategies and goals.
  • Maintain availability for emergency situations and provide on-call support as needed.

Job Requirements

  • Associate of Applied Science (AAS) degree in Nursing or related field.
  • Minimum of 1 year of nursing experience, preferably with developmental disability populations.
  • Proficiency in electronic health record systems and care coordination tools.
  • Strong knowledge of healthcare regulations and nursing standards.
  • Excellent supervisory and training capabilities to lead healthcare teams effectively.
  • Flexibility to adapt to hybrid work arrangements and on-call responsibilities.
