Overview

  • Lead clinical operations at a hospice care organization, ensuring exceptional patient care and compliance with healthcare standards.
  • Supervise a multidisciplinary team, fostering collaboration and professional development among clinicians.
  • Coordinate patient services to align with established hospice program policies and regulatory guidelines.
  • Conduct quality assurance activities, including chart audits and medical record reviews for compliance.
  • Develop and implement orientation and training programs for clinical staff to enhance their proficiency.
  • Evaluate staff performance, providing constructive feedback and counseling to improve service delivery.
  • Lead interdisciplinary team meetings to refine patient care plans and address challenges effectively.
  • Promote a positive work culture and support organizational growth initiatives.

Key Responsibilities & Duties

  • Coordinate and oversee patient services, ensuring alignment with hospice program policies and standards.
  • Provide leadership and guidance to a team of clinicians, fostering continuous improvement in care delivery.
  • Conduct chart audits and maintain medical records to ensure compliance with regulatory requirements.
  • Develop and deliver orientation and in-service training programs for clinical staff.
  • Evaluate clinical staff performance, offering feedback and counseling to enhance their skills.
  • Lead interdisciplinary team meetings to address patient care plans and foster collaboration.
  • Ensure adherence to Federal, State, and organizational guidelines in hospice care practices.
  • Support organizational growth and maintain a positive work environment.

Job Requirements

  • Bachelor's degree in nursing or healthcare preferred; BSN required.
  • Registered Nurse with a current state license to practice as a professional nurse.
  • Minimum of 3 years of nursing experience, including 1 year in hospice or home health care.
  • At least 1 year of supervisory experience in hospice or home health care preferred.
  • Proficiency in crisis intervention, pain management, and family counseling.
  • Computer proficiency, including experience with EMR systems.
  • Current CPR certification and valid driver's license with access to reliable transportation.
  • Strong leadership and emotional intelligence skills.
