Overview

- Conduct quality assurance activities, including chart audits and medical record reviews for compliance.
- Develop and implement orientation and training programs for clinical staff to enhance their proficiency.

- Lead interdisciplinary team meetings to refine patient care plans and address challenges effectively.

Key Responsibilities & Duties
- Coordinate and oversee patient services, ensuring alignment with hospice program policies and standards.
- Provide leadership and guidance to a team of clinicians, fostering continuous improvement in care delivery.
- Conduct chart audits and maintain medical records to ensure compliance with regulatory requirements.
- Develop and deliver orientation and in-service training programs for clinical staff.

- Ensure adherence to Federal, State, and organizational guidelines in hospice care practices.

Job Requirements
- Bachelor's degree in nursing or healthcare preferred; BSN required.
- Registered Nurse with a current state license to practice as a professional nurse.
- Minimum of 3 years of nursing experience, including 1 year in hospice or home health care.
- At least 1 year of supervisory experience in hospice or home health care preferred.
- Proficiency in crisis intervention, pain management, and family counseling.
- Computer proficiency, including experience with EMR systems.
- Current CPR certification and valid driver's license with access to reliable transportation.
- Strong leadership and emotional intelligence skills.
