Job Description

Overview

  • Provide skilled nursing care and support to patients in a healthcare setting.
  • Work on-site during the 7 AM to 3 PM shift with rotating weekends.
  • Ensure patient safety and satisfaction through high-quality care delivery.
  • Collaborate with Registered Nurses and Clinical Care Managers for patient management.
  • Perform charge nurse responsibilities as assigned by supervisors.
  • Commit to a minimum of three shifts per week with weekend rotations.
  • Utilize your expertise in geriatric care to enhance patient outcomes.
  • Contribute to a collaborative and professional healthcare environment.

Key Responsibilities & Duties

  • Administer medications and treatments as prescribed by healthcare providers.
  • Monitor patient conditions and report changes to the healthcare team.
  • Document patient care activities and maintain accurate medical records.
  • Provide emotional support and education to patients and their families.
  • Assist in the development and implementation of patient care plans.
  • Supervise and guide nursing assistants in their daily tasks.
  • Perform IV therapy and other specialized nursing procedures.
  • Ensure compliance with healthcare regulations and safety standards.

Job Requirements

  • Valid NYS LPN license and certification in BCLS and IV therapy.
  • Minimum of 1 year of experience working with geriatric patients.
  • Graduate of an approved program in practical nursing.
  • Ability to commit to rotating weekend shifts and a minimum of three shifts weekly.
  • Strong communication and organizational skills for effective patient care.
  • Proficiency in administering medications and performing specialized procedures.
  • Knowledge of healthcare regulations and patient safety standards.
