Job Description

Overview

  • Provide exceptional care and support to residents/patients in a healthcare setting.
  • Perform duties as a Licensed Practical Nurse (LPN) to ensure safety and satisfaction.
  • Work collaboratively with Registered Nurses and Clinical Care Managers.
  • Commit to a rotating weekend schedule with flexible shift options.
  • Contribute to a dynamic healthcare team in a fast-paced environment.
  • Ensure compliance with healthcare standards and protocols.
  • Engage with geriatric patients to provide compassionate and effective care.
  • Participate in ongoing training and development opportunities.

Key Responsibilities & Duties

  • Administer medications and treatments as prescribed by healthcare providers.
  • Monitor patient health and report changes to Registered Nurses or Clinical Care Managers.
  • Maintain accurate patient records and documentation.
  • Assist in developing and implementing patient care plans.
  • Provide emotional support and education to patients and their families.
  • Ensure adherence to infection control and safety protocols.
  • Collaborate with interdisciplinary teams to optimize patient outcomes.
  • Perform charge nurse responsibilities as assigned.

Job Requirements

  • Valid NYS Licensed Practical Nurse (LPN) license required.
  • Previous experience working with geriatric populations preferred.
  • Graduate of an approved program in practical nursing.
  • Basic Life Support (BCLS) and IV certification required.
  • High School Diploma or GED required.
  • Ability to commit to rotating weekend shifts.
  • Strong communication and interpersonal skills.
  • Proficient in healthcare documentation and patient care protocols.
