Overview
- Provide comprehensive nursing care and case management for individuals with developmental disabilities in a remote setting.
- Collaborate with interdisciplinary teams to ensure optimal health outcomes and compliance with OPWDD regulations.
- Supervise and train staff in nursing tasks, protective health measures, and medication administration.
- Develop and maintain individualized nursing care plans, updating them annually or as conditions change.
- Conduct regular nursing assessments and monitor health statuses, ensuring timely communication with clinical personnel.
- Utilize electronic health records and care coordination systems effectively to manage patient information.
- Participate actively in interdisciplinary team meetings to align care strategies and goals.
- Ensure compliance with State and Federal regulations in all nursing practices and documentation.
Key Responsibilities & Duties
- Provide direct nursing care as prescribed, adhering to recognized standards and regulations.
- Perform physical examinations and nursing assessments upon program entry and quarterly thereafter.
- Coordinate acute and ongoing health services based on individual needs and regulatory mandates.
- Monitor health statuses, lab results, and treatment efficacy, ensuring accurate documentation.
- Train staff in health and hygiene methodologies, communicable disease control, and client-specific medical issues.
- Supervise LPNs and direct support professionals, conducting annual clinical performance evaluations.
- Maintain training records and ensure staff readiness for client-specific assignments.
- Participate in hiring and scheduling of LPN staff, ensuring optimal team performance.
- Provide on-call support as needed, ensuring continuity of care and emergency readiness.
Job Requirements
- Associate of Occupational Studies (AOS) degree required, with relevant nursing certifications preferred.
- Minimum of 1 year of nursing experience, preferably with OPWDD or MRDD populations.
- Proficiency in electronic health record management and care coordination systems.
- Strong knowledge of OPWDD regulations and compliance standards.
- Ability to supervise and train staff in nursing tasks and health measures.
- Excellent communication skills for interdisciplinary collaboration and patient interaction.
- Flexibility in scheduling and availability for on-call support as needed.
- Commitment to maintaining accurate and timely medical documentation.
