Key Responsibilities & Duties

  • Provide comprehensive care for patients with acute and chronic conditions, emphasizing preventive measures.
  • Lead and mentor a care team, fostering collaboration and patient-focused service delivery.
  • Coordinate with specialists and hospitalists to ensure seamless patient care transitions.
  • Document patient interactions and treatment plans using advanced healthcare systems.
  • Participate in clinical meetings and quality improvement initiatives to enhance care standards.
  • Supervise Advanced Practice Practitioners (APPs), ensuring adherence to evidence-based practices.
  • Promote lifestyle interventions and preventive care for improved patient outcomes.
  • Support operational objectives and assist in coverage for other providers as necessary.
  • Engage in community outreach and recruitment efforts to expand healthcare access.

Job Requirements

  • Doctor of Medicine (MD) degree with board certification in Internal Medicine, Family Medicine, or Geriatrics.
  • Active State Medical License or eligibility to obtain one promptly.
  • Current DEA registration for prescribing controlled substances.
  • Basic Life Support (BLS) certification within 90 days of employment.
  • Minimum of 1 year of clinical experience, with 5 years preferred in primary care settings.
  • Proficiency in healthcare documentation systems and Microsoft Office Suite.
  • Strong commitment to patient-centered care and community-focused healthcare delivery.
  • Ability to travel locally and nationally for training and recruitment initiatives.
  • Fluency in English, with additional language skills preferred for specific patient populations.
