Overview
- Provide comprehensive primary and preventive care to adult patients in a culturally diverse community-based health center.
- Diagnose and manage acute and chronic medical conditions while promoting overall wellness and patient education.
- Collaborate with multidisciplinary teams to ensure coordinated, patient-centered care delivery.
- Maintain accurate and timely documentation within the electronic health record (EHR) system.
- Participate in quality improvement initiatives and contribute to team meetings and discussions.
- Engage with the community to address healthcare needs and promote preventive care strategies.
- Support the health center's mission by delivering culturally competent care to a diverse patient population.
- Provide guidance on lifestyle modifications, disease prevention, and medication management.
Key Responsibilities & Duties
- Conduct physical examinations, screenings, and chronic disease management for adult patients.
- Diagnose and treat acute and chronic medical conditions, ensuring optimal patient outcomes.
- Collaborate with multidisciplinary teams to develop and implement patient-centered care plans.
- Educate patients on lifestyle modifications, disease prevention, and medication adherence.
- Maintain accurate and timely documentation in the electronic health record (EHR) system.
- Participate in quality improvement initiatives to enhance healthcare delivery and patient satisfaction.
- Engage in team meetings and contribute to the development of best practices in patient care.
- Perform additional tasks as directed by the supervisor to support the health center's operations.
Job Requirements
- Master of Science in Nursing with specialization in Internal Medicine or Adult Primary Care.
- Current New York State license or eligibility for licensure is required.
- Minimum of 2 years of experience in community health or outpatient primary care.
- Preferred 4 years of experience in Internal Medicine or Adult Primary Care settings.
- Strong communication and interpersonal skills to effectively interact with patients and teams.
- Bilingual proficiency in Creole and English is strongly preferred.
- Ability to work on-site in a community-based health center environment.
