Overview

  • Provide patient-centered primary care services, emphasizing preventive care and chronic disease management in a supportive healthcare environment.

  • Prescribe medications responsibly, adhering to clinical guidelines and state regulations to ensure patient safety.

Key Responsibilities & Duties

  • Conduct thorough assessments of patients to identify health concerns and develop treatment plans.
  • Implement individualized care strategies in collaboration with physicians and healthcare professionals.
  • Monitor and adjust care plans for patients with complex medical conditions, ensuring optimal outcomes.
  • Educate patients and families on disease management, preventive care, and treatment options.
  • Maintain accurate documentation using electronic medical records (EMR) systems.

Job Requirements

  • Doctorate of Nursing Practice (DNP) with active board certification in relevant specialties.
  • Current, unrestricted Nurse Practitioner license in the state of practice.
  • DEA licensure for prescribing controlled substances.
  • Minimum of 1 year clinical experience, with 3 years preferred, in adult, family, or geriatric care.
  • Proficiency in managing patients with advanced illnesses and chronic conditions.
  • Experience in long-term care settings and interdisciplinary team collaboration.
  • Knowledge of electronic medical records (EMR) systems.
  • Psychiatric experience is advantageous but not mandatory.
