Job Description

Overview

  • Lead a team of clinicians in providing exceptional hospice care services.
  • Coordinate interdisciplinary efforts to ensure comprehensive patient and family support.
  • Drive compliance with federal, state, and accreditation standards.
  • Promote a culture of growth and excellence within the hospice program.
  • Collaborate with leadership to implement strategic initiatives and improve patient outcomes.
  • Ensure adherence to ethical care benchmarks and regulatory guidelines.
  • Provide mentorship and development opportunities for clinical staff.
  • Oversee medical record maintenance and quality assurance processes.
  • Participate in crafting orientation and training programs for clinical staff.

Key Responsibilities & Duties

  • Coordinate patient services to align with hospice program policies.
  • Provide leadership and support to clinical teams for service improvement.
  • Conduct chart audits and ensure compliance with certification standards.
  • Evaluate clinical staff performance and provide constructive feedback.
  • Lead interdisciplinary team meetings to refine care plans.
  • Develop and implement training programs for clinical staff.
  • Collaborate with leadership to foster growth in patient numbers.
  • Ensure compliance with hospice guidelines and regulations.
  • Provide crisis intervention and support for patients and families.

Job Requirements

  • Bachelor of Science in Nursing (BSN) from an accredited program preferred.
  • Current unencumbered Registered Nurse license required.
  • Minimum three years of direct patient care experience, with one year in a supervisory role.
  • Proficiency in crisis intervention, pain management, and family counseling.
  • Strong computer skills, including experience with electronic medical records (EMR).
  • Current CPR certification and valid driver’s license required.
  • Demonstrated ability to lead and manage clinical teams effectively.
  • Knowledge of hospice care regulations and accreditation standards.
  • Exceptional communication and organizational skills.
