Overview

Key Responsibilities & Duties
- Conduct an average of six patient visits per day in home settings.
- Perform wound care and other skilled nursing interventions with precision.
- Complete comprehensive patient assessments and monitor their progress.
- Manage patient admissions and intakes, ensuring accurate documentation.
- Collaborate with care teams to develop and implement effective care plans.
- Demonstrate strong clinical judgment and confidence in independent decision-making.
- Provide compassionate care to patients and their families, addressing their concerns.
- Participate in professional development opportunities to enhance skills.
Job Requirements
- Associate of Applied Science (AAS) in Nursing or equivalent required.
- Minimum of 1 year of clinical experience, preferably with CHHA population.
- Proficiency in wound care and skilled nursing interventions preferred.
- Strong clinical judgment and ability to work independently in field settings.
- Effective collaboration skills to ensure continuity of patient care.
- Flexibility to participate in weekend rotations as required.
- Commitment to delivering compassionate care and maintaining patient dignity.
