Overview

  • Provide high-quality in-home clinical care to patients, ensuring their health and comfort.
  • Work independently in a remote field-based setting with flexible scheduling.

Key Responsibilities & Duties

  • Conduct an average of six patient visits per day in home settings.
  • Perform wound care and other skilled nursing interventions with precision.
  • Complete comprehensive patient assessments and monitor their progress.
  • Manage patient admissions and intakes, ensuring accurate documentation.
  • Collaborate with care teams to develop and implement effective care plans.
  • Demonstrate strong clinical judgment and confidence in independent decision-making.
  • Provide compassionate care to patients and their families, addressing their concerns.
  • Participate in professional development opportunities to enhance skills.

Job Requirements

  • Associate of Applied Science (AAS) in Nursing or equivalent required.
  • Minimum of 1 year of clinical experience, preferably with CHHA population.
  • Proficiency in wound care and skilled nursing interventions preferred.
  • Strong clinical judgment and ability to work independently in field settings.
  • Effective collaboration skills to ensure continuity of patient care.
  • Flexibility to participate in weekend rotations as required.
  • Commitment to delivering compassionate care and maintaining patient dignity.
  • Eligibility for pension plan with 7% employer contribution after 3 years.
