Registered Nurse Clinical Hospice Liaison

Key Responsibilities & Duties

• Build and maintain relationships with healthcare providers to promote hospice services.
• Conduct outreach activities to identify new referral sources and increase patient census.
• Educate patients and families about hospice care philosophy and services.
• Collaborate with medical staff to determine patient eligibility for hospice services.
• Develop and implement strategies to achieve growth and census management goals.
• Conduct regular visits to healthcare facilities to maintain relationships and secure referrals.
• Ensure compliance with hospice standards and regulations in all outreach activities.
• Provide exceptional customer service to referral sources, patients, and families.

Job Requirements

• Bachelor of Science in Nursing (BSN) required; advanced degree preferred.
• Current, unencumbered Registered Nurse (RN) license mandatory.
• Minimum of 1 year of healthcare sales or marketing experience required.
• Preferred 1-2 years of hospice or palliative care marketing experience.
• Proven ability to establish and maintain relationships with healthcare professionals.
• Reliable transportation, valid driver’s license, and auto insurance required.
• Strong communication and empathetic skills to educate and support patients and families.
• Ability to work independently and collaboratively in a hybrid work environment.
