Job Description

Overview

  • Manage care coordination for high-risk patients, ensuring effective treatment plans and improved health outcomes.
  • Support self-management strategies to empower patients in managing their health conditions.
  • Assist in reducing complications, readmissions, and overall healthcare costs through proactive care management.
  • Provide leadership by supporting supervisors, conducting audits, and training new associates.
  • Ensure patients remain eligible for services and progress toward their healthcare goals.
  • Collaborate with multidisciplinary teams to deliver comprehensive care solutions.

Key Responsibilities & Duties

Job Requirements
