Overview
- Provide compassionate social work services to adult and geriatric patients in an inpatient setting, ensuring seamless care transitions and support.
- Coordinate discharge planning and referrals to post-acute care, rehabilitation, and community-based services.
- Collaborate with interdisciplinary teams to deliver holistic patient care and support.
- Utilize advanced systems for patient documentation and care coordination activities.
- Conduct needs assessments to arrange durable medical equipment and home care services.
- Educate patients and caregivers on discharge plans and available community resources.
- Support the operational needs of the Case Management Department through effective collaboration and communication.
Key Responsibilities & Duties
- Manage a caseload of patients, ensuring timely and effective care coordination and discharge planning.
- Perform pre-certification reviews and insurance authorizations to facilitate patient care transitions.
- Collaborate with healthcare teams to ensure comprehensive care delivery across various acute care settings.
- Provide education to patients and caregivers regarding discharge plans and community resources.
- Coordinate home care services and durable medical equipment needs post-discharge.
- Support emergency department, ICU, NICU, and other acute care settings as required.
- Ensure compliance with Medicaid notices and manage insurance authorizations effectively.
Job Requirements
- Master of Social Work degree required with MSW certification.
- Minimum 2 years of experience in case management or inpatient medical social work preferred.
- Proficiency in patient documentation and care coordination systems.
- Knowledge of Medicaid, Medicare, and community-based service programs beneficial.
- Experience in acute care settings such as ICU, NICU, and emergency departments preferred.
- Strong skills in discharge planning, needs assessments, and referral coordination.
- Ability to collaborate effectively with interdisciplinary teams.
