Overview
- Support home care operations by managing patient intake processes, ensuring accuracy and compliance.
- Serve as a key contact for patients and families, facilitating communication and coordination.
- Handle referrals from hospitals, physicians, and community sources efficiently.
- Ensure HIPAA compliance and maintain confidentiality in all operations.
- Collaborate with clinical, administrative, and billing teams in a fast-paced environment.
- Provide administrative support and resolve intake issues promptly.
- Track and report intake metrics to improve operational efficiency.
- Utilize systems like HHA Exchange and ePaces for data management.
- Contribute to the Start of Care Bonus Program by managing SOCs effectively.
Key Responsibilities & Duties
- Manage the full intake process for new home care patients, ensuring data accuracy.
- Collect and verify patient demographics and insurance information.
- Conduct intake interviews and gather required documentation efficiently.
- Follow up on incomplete referrals and pending documents proactively.
- Handle high call volume professionally, addressing inquiries effectively.
- Resolve intake issues and escalate complex cases as needed.
- Maintain HIPAA compliance and confidentiality in all interactions.
- Provide general administrative support to enhance team operations.
- Collaborate with cross-functional teams to ensure seamless patient onboarding.
Job Requirements
- Bachelor of Science (BS) degree is required.
- Minimum of 1 year and preferred 3 years of home care intake experience.
- Proficiency in English and Spanish (spoken and written) is mandatory.
- Experience with HHA Exchange and familiarity with ePaces or Zoho preferred.
- Knowledge of Medicare and Medicaid regulations is advantageous.
- Strong data entry skills and attention to detail are essential.
- Proficiency in Microsoft Office and standard office systems is required.
- Ability to multitask and work in a fast-paced environment effectively.
- Must be able to work on-site and commute to the designated location.
