Overview
- Provide essential healthcare services in a 35-bed residential program supporting individuals recovering from mental health and substance use challenges.
- Collaborate with an interdisciplinary team to enhance residents' health outcomes and overall quality of life.
- Engage directly with residents to promote wellness, independence, and health education.
- Perform medical case management and coordinate care with healthcare providers and pharmacies.
- Document healthcare activities, maintain medication records, and ensure compliance with health regulations.
- Participate in treatment planning and case conferences to address resident health needs.
- Respond to medical emergencies and provide immediate care as required.
- Supervise staff and contribute to the smooth operation of the healthcare program.
Key Responsibilities & Duties
- Conduct monthly health-focused sessions to identify resident health goals and promote independence.
- Facilitate health and wellness groups to educate residents on maintaining their health.
- Perform health assessments, vital sign monitoring, and ongoing evaluations of resident health status.
- Coordinate care with physicians, pharmacies, and treatment providers to ensure comprehensive healthcare delivery.
- Document client care activities, maintain accurate medication administration records, and oversee controlled substance logs.
- Advocate for resident health needs and provide health education to support wellness goals.
- Respond promptly to medical emergencies and administer first aid or CPR as needed.
- Participate in team meetings and case conferences to align on resident care strategies.
Job Requirements
- Active New York State Licensed Practical Nurse (LPN) license is mandatory.
- Associate of Applied Science (AAS) degree or equivalent educational background required.
- Minimum of 1 year of experience in healthcare; 3 years preferred.
- Experience working with behavioral health, substance use, or residential populations is advantageous.
- Strong communication skills and ability to coordinate care effectively.
- Proficiency in documenting healthcare activities and maintaining accurate records.
