Job Description

Overview

  • Provide multidisciplinary health services for older adults in a supportive day care environment.
  • Coordinate care plans with physicians and healthcare team members to ensure optimal patient outcomes.
  • Administer medications and perform skilled nursing services as per physician orders.
  • Conduct comprehensive clinical assessments and document findings in patient records.
  • Monitor patient progress and adapt care plans to meet changing needs.
  • Educate patients and families on disease management and preventive care strategies.
  • Participate in multidisciplinary meetings to review patient care and outcomes.
  • Ensure compliance with insurance plans and physician-approved care strategies.
  • Maintain accurate documentation of all services provided and communications.

Key Responsibilities & Duties

  • Conduct intake and quarterly clinical assessments to identify patient service needs.
  • Document nursing evaluations, care plans, and follow-up recommendations in clinical records.
  • Ensure timely completion and receipt of physician orders.
  • Administer medications and provide skilled nursing services as per physician directives.
  • Assign and oversee completion of tasks by Certified Nursing Assistants (CNAs).
  • Collaborate with healthcare team to align clinical services with care plan goals.
  • Evaluate patient diagnoses and provide input on care plan interventions.
  • Educate patients and families on self-care techniques and disease prevention.
  • Maintain written and verbal communications to ensure optimal patient care coordination.

Job Requirements

  • Current license and registration to practice as a Registered Nurse in New York State.
  • Bachelor of Science in Nursing (BSN) from an accredited institution preferred.
  • Minimum of one year experience in acute or subacute care settings.
  • Experience in adult care centers and geriatric care preferred.
  • Bilingual proficiency in Spanish and English is advantageous.
  • UAS certification is preferred for this role.
  • Strong documentation and communication skills are essential.
  • Ability to work on-site and adhere to scheduled hours.
