Overview

  • Lead clinical operations for a home healthcare agency, ensuring compliance with regulations and delivering high-quality patient care.
  • Coordinate patient care needs and develop individualized treatment plans in collaboration with physicians and field staff.
  • Provide oversight to clinical staff, ensuring accurate documentation and adherence to agency protocols.
  • Serve as a resource for patients, families, and referral parties, promoting effective communication and cultural sensitivity.
  • Participate in quality improvement initiatives and staff orientation programs to enhance service delivery.
  • Maintain confidentiality and demonstrate commitment to customer relations and quality service standards.
  • Utilize community resources to meet patient care needs and support timely discharge planning.
  • Participate in on-call supervisor rotation, ensuring continuity of care and support for field staff.

Key Responsibilities & Duties

  • Coordinate and assess home healthcare needs of referred patients, ensuring timely intake and discharge planning.
  • Collaborate with healthcare professionals to establish appropriate care levels and comprehensive treatment plans.
  • Order necessary medical supplies and services, ensuring proper documentation and compliance.
  • Conduct case conferences to ensure effective case management and coordination among clinical personnel.
  • Review admission and recertification documentation to confirm alignment with patient needs and care plans.
  • Demonstrate knowledge of reimbursement practices and state and federal regulations.
  • Provide guidance and direction to support staff, ensuring continuity of care and adherence to protocols.
  • Participate in performance improvement activities and staff orientation programs to enhance service quality.

Job Requirements

  • License to practice as a registered nurse in the state; BSN preferred.
  • Minimum of three years of nursing experience, with two years in home health preferred.
  • Knowledge of OASIS and clinical documentation standards is essential.
  • Strong verbal and written communication skills to interact effectively with patients and staff.
  • Excellent problem-solving and organizational skills to manage multiple responsibilities.
  • Commitment to customer relations and maintaining confidentiality at all times.
  • Ability to demonstrate cultural sensitivity and utilize community resources effectively.
