Speech Language Pathologist

Overview

• Provide speech therapy services in a home care setting, ensuring quality care for diverse patient populations.
• Conduct evaluations, develop treatment plans, and deliver therapy to improve patient outcomes.
• Collaborate with interdisciplinary teams to maintain continuity of care and address patient needs.
• Utilize company-provided technology for documentation and communication with clinical leadership.
• Participate in daily team huddles and periodic clinical meetings for updates and training.
• Work independently in the field while maintaining communication with scheduling teams.
• Access office resources for printing, meetings, and additional support as needed.
• Ensure patient safety through adherence to established protocols and use of monitoring systems.

Key Responsibilities & Duties

• Conduct comprehensive speech therapy evaluations and assessments in patients' homes.
• Develop individualized treatment plans based on patient needs and therapy goals.
• Provide ongoing therapy services to improve communication and swallowing functions.
• Coordinate with interdisciplinary teams to ensure holistic patient care.
• Complete accurate documentation using company technology platforms.
• Maintain productivity standards through effective scheduling and patient management.
• Participate in clinical meetings and training sessions for professional development.
• Adhere to safety protocols and utilize field-based support resources as needed.

Job Requirements

• Bachelor of Science in Speech-Language Pathology or related field.
• Active Speech Language Pathologist license required.
• Minimum of 1 year of experience in speech therapy; 3 years preferred.
• Experience in home care or community-based healthcare is advantageous.
• Reliable transportation and willingness to travel within assigned territory.
• Proficiency in using technology for documentation and communication.
• Strong communication and relationship-building skills with patients and teams.
• Ability to work independently while adhering to clinical guidelines and protocols.
