Job Description

Overview

  • Provide nursing care and medical procedures within the scope of an LPN license.
  • Assist in integrating physical and behavioral health in program settings.
  • Collaborate with RNs and nursing leadership to ensure quality care delivery.
  • Administer prescribed medical treatments and adhere to documentation policies.
  • Coordinate appointments and liaise between healthcare providers and community organizations.
  • Respond promptly to medical emergencies and perform necessary triage.
  • Perform phlebotomy and specimen preparation for laboratory analysis.
  • Maintain accurate patient charting and comply with regulatory standards.

Key Responsibilities & Duties

  • Administer prescribed medications and document administration details accurately.
  • Monitor and record clients' vital signs and report adverse reactions.
  • Coordinate medical appointments and manage relevant documentation.
  • Act as a liaison between external healthcare providers and the facility.
  • Respond to emergencies and provide immediate medical attention.
  • Ensure compliance with internal and external healthcare policies.
  • Participate in quality improvement projects for enhanced client outcomes.
  • Maintain inventory and requisition necessary medical supplies.

Job Requirements

  • Active Licensed Practical Nurse (LPN) license in New York State.
  • Current CPR certification is mandatory.
  • Associate of Applied Science (AAS) degree required.
  • Minimum of 1 year of relevant nursing experience preferred.
  • Proficiency in oral and written communication skills.
  • Ability to manage data and maintain organized documentation.
  • Capability to respond swiftly and effectively to medical emergencies.
  • Physical stamina to perform nursing duties throughout the shift.
